Applications for
2020-2021 Pre-Kindergarten

Your child must be 4 years old on or before
Aug. 15, 2020, to attend 2020-2021 Pre-K at MES.

You may print the following documents at home.

(You must also bring the additional documents listed below.)

Completing the Pre-K application packet does NOT
guarantee enrollment in the program.

Parents should bring the following documents:

If you are unable to provide any of the requested documents,
please contact your zoned school principal.


1.  child’s certified birth certificate (not the mother’s copy)

2.  two proofs of residency within the MES school zone

The following documents are acceptable proof of residency (all documents must be in the parent's name and include the address): property tax records; mortgage document or property deed; apartment or house rent/lease agreement; current utility bill; automobile registration; or voter registration card.

NOTE: Cable bills and phone bills are NOT acceptable proof of residency.


3.  proof of income

The following documents are acceptable proof of income: recent pay stub/verification from employer; 2017 W-2 form; 2017 income tax form 1040A or 1040; unemployment compensation; workman's compensation; pension; retirement documentation; Social Security; veteran's benefit letter; child support; alimony; SSI; TennCare verification; or documentation of participation in the following programs: Early Head Start; Head Start; Homeless; Families First (TANF); Federal Even Start Family Literacy; Foster Care; Migrant; Food Stamps/EBT.

4. Tennessee Certificate of Immunization form

with completed Health Examination Documentation (top right corner of form)
